SWAN, Victoria - Suddenly at the Pembroke Regional Hospital on Monday, November 28th, 2016. Cherished daughter of Adam Swan and Deidra Swan Chartrand. Sadly missed by sister Eleanor, by grandparents Tom and Cathy Chartrand, and Adrian and Patricia Swan. Respecting the family wishes, cremation has taken place.
